1. Ask your physician to fax a referral* to 616-741-2310.

Once we receive a referral we will contact you.

*A medical provider's referral is required to schedule an evaluation. An order must have the client's name, date of birth, primary diagnosis, and state "Occupational Therapy (OT) driver evaluation and treatment." Visit the Referrals page for more information.
